So, firstly I would say this. Most businesses measure the wrong number.
You can see sick days. They show up in a system, someone is absent, you count it. So that is the number that gets the attention.
But here is the thing. The days someone is off sick are the small part of the problem. The bigger cost is the person who is at their desk, but running at half power.
BLUF: that is presenteeism. People physically present but underperforming through poor health, tiredness or low energy. And it is roughly 90% of the productivity you are losing.
That being said, because you cannot see it on a dashboard, it usually gets ignored. Which is exactly why it is so expensive.
Think of it like an iceberg
Absenteeism is the tip above the water. Visible, countable, and only about 10% of the total.
Presenteeism is the 90% below the surface. The afternoon slump. The foggy head. The person who is technically working but getting a fraction done, making slower decisions, and dropping the odd ball they normally would not.
For example: someone who sleeps badly, eats on the run, and has not moved properly in months does not call in sick. They just quietly operate at 70%. Every day. And you are paying their full salary for it.

In the UK, the average employee loses close to 49.7 days of productive time a year to ill health (Vitality). Around 90% of that is presenteeism, not absence.
Presenteeism is estimated to cost UK employers over £21 billion a year, and it costs businesses two to three times more than absence does.
The key message here is: the cost you can measure is the smaller one. The cost you cannot see is where the money actually goes.
Why this is good news, not bad
Here is the part I want you to sit with. You are already paying for this.
The salaries are already going out of the door. So recovering even part of that lost capacity is not new spend, it is money you are currently losing, coming back.
And the biggest levers on presenteeism are the physical ones. Physically inactive employees lose about 28% more productive time than active ones, and poor diet adds more on top (Vitality). Fitness, energy, sleep and nutrition are not "soft" wellbeing. They are the direct drivers of whether someone shows up at 70% or 95%.
What to do about it
Here's what to do:
- Stop measuring only absence. If sick days are your only health metric, you are managing 10% of the problem. Ask how your people actually feel and function.
- Target the physical drivers. Fitness and nutrition move presenteeism in a way a fruit bowl never will.
- Make it coached, not passive. People need a plan and support, not another perk they ignore.
- Put a pound figure on it. When you can show the finance team the number, the conversation changes.
